That 2066 number is my staples rewards premier number, but when you click on the link to apply for the card, there is a spot to put in your staples rewards card number, if you know it/have one. It's not a required field.

If you go to the staples website and click on Staples Credit Center (bottom left), it takes you to the same enrollment form w/ Citi.
