To get your $50 Midwest Savings Certificate, just follow these four steps.
1.) Open a new North Shore Bank checking account at any of our locations or online.
2.) You'll receive a Registration Form from North Shore Bank. Complete your name, address, signature and date on the Registration Form.
3.) Mail your Registration Form by 12/31/04 to:
