One of my close friend was a victim of "phishing" through which his ATM card details were hacked, it happened in the form of an e-mail from the bank which issued his ATM card specifying of an important security update to the ATM card using the form when he clicks, ignorantly he entered all the details and there was a thankful message for using this form, he did not realize that the form was hosted on the hacker website and not on the bank website, after realizing his mistake he immediately inform the bank to stop any trasactions through his ATM card, thankfully no damage was done.
One of the most important trick to identify an "online phishing message" is to verify whether it is displayed on the bank website and if it is displayed then whether the message is hosted on the secured page of the bank website and all the links in the page leads to the bank internal pages only and not to other third party websites.
